Services rendered are considered Out-of-Network for insurance claims. You will be provided with a detailed insurance reimbursement receipt to submit to your insurance company for possible reimbursement. Please be advised that insurance reimbursement rates may vary widely depending on your insurance provider, your individual plan, your benefits and your deductible. It is recommended that you contact your insurance company prior to pursuing any diagnostic or treatment services to become familiar with your benefits and understand your financial obligation.
